Job Description:
We are currently recruiting for an Independent Living Advisor who will work remotely and travel to appointments across Bristol and South Gloucestershire.

You will support a caseload of customers with a wide range of disabilities and communication needs who are considering/choosing to participate in a Self-Directed Support Scheme, Independent Living Scheme or Personal Health Budget and you will also support self-funded/private clients.

You will provide a person-centered approach to support your customers to retain their independence, choice, and control through establishing a care and/or support plan. You will visit customers in their homes, local authority offices and community locations and you will help each customer to manage their obligations and responsibilities in relation to their individualised care package.

You will collate accurate reports daily, ensuring compliance is always in place and that all information is accurate, clear and in a format which meets the customers individual needs.

Due to the nature of this role, you will be a car driver with access to your own vehicle and you will be required to complete an online Disclosure and Barring Service (DBS) check.
