This role provides an exciting opportunity to work with young care leavers across the North West, helping them develop as individuals and progress within their communities.
Monday - Friday with alternative weekends

Based between Blackburn and Atherton - travel across sites
Working and travelling to clients across the North West.
Aims of post:
To provide a quality Floating Support Housing service for young people who have low, medium or high support needs, and/or may have complex, chaotic or multiple support needs.
To enable clients to develop their skills and networks they require to move on and live independently in their own accommodation.
To develop and deliver client centred support plans that incorporate specific identified smart outcomes and activities to achieve these that are tailored to individual client needs.
To act as an advocate for the client and to work in partnership with local agencies to meet individual clients support needs and achieve positive outcomes for them.
To formulate, co-ordinate and deliver intervention strategies when clients are in a crisis situation.
To assist the Service Manager in marketing and promoting the service to other agencies and organisations.
To support staff in the Blackburn and Atherton service when required.
An enhanced DBS certificate including clearance to work with children is a necessity for the role.
