Partnership Neighbourhood Model Program Lead needed in Barnet £355.87 per day PAYE - Reference: RQ680776
This role will involve:
- The post holder will lead the design, planning and implementation of the neighbourhood model programme for the Barnet Borough Partnership as an initial priority, and other programmes of work as the role and BBP priorities evolve over time.
- Support NCL CCG and its partners to deliver a meaningful reduction in health inequalities and a system shift towards person centred care, prevention, early intervention, self-care and self-management.
- Works with commissioners and providers to ensure the processes and incentives in the system, support integration not fragmentation or silo working (for example, approaches to planning, service (re)design, business case development, payment models, contracting models, performance frameworks etc.)
This is a Full time role on a temporary contract basis.
