Job Description:
We are recruiting for a Work and Health Hub Guide to support the delivery of employability services within Health and Wellbeing Hubs across Manchester. This is an excellent opportunity for an enthusiastic and proactive individual who enjoys building relationships, engaging with communities and helping people access the support they need to move closer to employment.
Working within a healthcare setting, you will raise awareness of the service, generate referrals, conduct eligibility and triage conversations, and provide participants with a positive introduction to their employability journey. You will work closely with healthcare professionals, community organisations and wider service teams to ensure individuals receive the right support at the right time.
The role involves:
- Promoting employability and wellbeing services across hospital sites through presentations, events, drop-in sessions and awareness campaigns
- Building relationships with healthcare professionals, hospital departments and partner organisations to generate referrals
- Working towards monthly referral and participant engagement targets
- Delivering outreach activities including telephone engagement, networking events, roadshows and face-to-face promotion
- Conducting triage, eligibility and onboarding conversations with potential participants
- Completing initial assessments and required programme documentation accurately
- Signposting and referring individuals to appropriate support services and community resources
- Working collaboratively with Health Hub teams, Integration Coordinators and Work & Health Coaches to ensure effective participant handovers
- Maintaining accurate records and ensuring compliance with programme requirements
You’ll need:
- Experience of promoting services and generating referrals through engagement and partnership working
- Experience building productive relationships with a range of stakeholders, including healthcare professionals, community groups or partner organisations
- Strong communication and interpersonal skills with the ability to build rapport quickly
- Confidence conducting telephone and face-to-face conversations with a diverse range of people
- Understanding of the barriers faced by individuals with health conditions, disabilities or other challenges affecting employment
- Experience organising and coordinating events, awareness campaigns or engagement activities
- Ability to manage a varied workload and prioritise effectively in a target-driven environment
- Strong organisational and administrative skills with attention to detail
- Ability to work independently and as part of a multidisciplinary team
- Experience within healthcare, wellbeing, employability or community services would be advantageous
- Ability to speak a community language relevant to Greater Manchester would be beneficial
